Case Management:
Majority of case managers provide specific assistance in certain situations such as homelessness or planning hospital discharge. They may work for agencies that only help a certain demographic, this may include frail elders, at-risk youth, or developmental disabilities. They may also assist with specific tasks, such as in-home care coordination or facility placement. Case managers who help coordinate care for frail or disabled elderly develop care plans based on individual needs. They may also assist with service delivery, finding resources, and/or monitoring progress.
